Minors will require a signed parental/guardian consent form before the first day of the course. Form provided a few weeks prior to start.

COVID-19 *

I understand that if I am not up to date on my COVID vaccination, including the recommended booster, I may be required to wear an N95 mask during my clinical rotation.

Course Format *
